Find a Provider
This is where you can find the name or location of a provider or hospital.
Instructions:
Search by last name. Fill in the last name of the provider or hospital
you are looking for.
Search by location. Fill in a ZIP code. To search in a general
area, members can enter the ZIP code and select how many miles around the ZIP code
you would like to search. You can also search by city.
Search by specialty. To find a provider by specialty, you can use
the drop down box.
