We are pleased to offer our newest health plan, Delaware Physicians Care Advantage, for those who are enrolled in both Medicare and Medicaid and live in New Castle county. Delaware Physicians Care Advantage pays for the following benefits NOT paid for by Medicare:

• Care Management Programs – You’ll be assigned a nurse who will act as your partner in health care, to help you manage and improve your health.

Delaware Physicians Care Advantage has a contract witih the Centers for Medicare and Medicaid (CMS). This contract renews each year. At the end of the year, the contract is reviewed and either Delaware Physicians Care Advantage or CMS can decide to end it. If this happens, you will receive a 90 day advance notice.
